WHAT TO BRING CHECKLIST

Below is a list of common items and information that will be needed during your appointment with the director at the funeral home.

  • Clothing (including undergarments)
  • Social Security Number
  • Date and place of birth
  • Full name of both parents (including mother's maiden name)
  • Educational background
  • Work history
  • Marital status (spouse's full name)
  • Names of children, grandchildren, great-grandchildren, siblings and spouses
  • Veterans discharge papers
  • Recent photograph (cosmetic purposes)
  • Religious affiliation with clergy name and phone number
  • List of pallbearers (if applicable)
  • A collection of valued photographs (30-50) for DVD Memorial Tribute
  • Payment (life insurance policies, check or credit card)
